Kiddie Ride Application
We Promote a litter free environment. No Dogs Allowed on Property.
The annual Swine Time Festival is held the Saturday after Thanksgiving. We can 
accommodate (2) Pony Rides, (2) Climbing Towers/Rock Wall and/or Spider Mountain
(2) Bungy Jumps, (1) Spider Jump, (1) Miniature Train Ride, (1) Inflatable Slide,
(1) Petting Zoo and other rides or events approved by Ride Coordinator.

We can accommodate two Space/Star/Moon walks, two Pony Rides, one Miniature Train
Ride and one Large Inflatable Slide, and not more than two Climbing Towers.

If you would like to participate, please complete the application below & send it
with a self addressed stamped envelope & cashiers check or money order made out to
the Climax Community Club for $200.00 per ride or event to:

No Electrical Power will be supplied for Ride Vendors. You must use a Generator.

Entries will be accepted on a "First Come Basis". It is recommended that you enter
your event as soon as possible after July 1.  If we are filled, your money order
will be returned in your SASE and we will place your name on the waiting list.
Include your phone no., so we can call if there is a cancellation.

Insurance is required for your entry: Please include a copy of your Declaration Page
that includes the Amount of Insurance, Company Name & Expiration Date.(Min. $1,000,000
each occurrence, Personal & ADV Injury, General Aggregate $50,000, Fire Damage $5,000 Med)

By signature above, Applicant accepts responsibility for any accident or mishap
associated with their Kiddie Ride or other Entertainment entry and agrees to
hold the Climax Community Club and The Swine Time Festival harmless from any
litigation resulting from any accident or mishap associated with applicant’s
entry.  Applicant certifies understanding of rules and will abide by the
guidelines set by the Climax Community Club and Swine Time Festival Committee.
No refund for any reason after October 1st of each calendar year.
